Community Health Care, Inc. - Established in 1975, CHC has been providing quality helth care for over 40 years!  CHC is one of the largest Federally Qualified Health Centers in the Midwest.  Nestled on the banks of the Mississippi River, in a region known as the Quad Cities, we serve a combined metro-area of over 400,000 residents.  CHC operates Medical and Dental facilities in Iowa and Illinois.  Our seven clinic locations are located in Davenport, IA, Moline, IL, Rock Island, IL, East Moline, IL and Clinton, IA.
