We are pleased to offer patients our CHC iConnect services, a telehealth option for primary care. The addition of this virtual appointment service has allowed CHC to better fulfill our mission of providing the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.

Who is eligible for virtual appointments:

  • New and established CHC patients may utilize our iConnect virtual care services

  • Telehealth is not just for acute care.  Any type of doctor's visit can start with telehealth.

  • Patients of all ages are eligible. Children should be accompanied by a parent/guardian during  appointments

  • Translation services are available for Spanish, Vietnamese and several other languages

How it works

  • The process is simple.  All iConnect appointments are scheduled by calling our Scheduling Department at (563) 336-3000

  • Our CHC iConnect Virtual Clinic is powered by our partners at OTTO Health.  Once your iConnect appointment is scheduled you will receive a link and login information for your virtual appointment.

Insurance & Billing:

  • We accept most private insurance, Iowa and Illinois Medicaid, Medicare and Non-insured patients

  • All of our patients can apply to receive assistance through our Sliding Fee Program that allows patients to pay for their services based on their income and family size. View more Sliding Fee Program inf here
