COMMUNITY HEALTH CARE, INC. RECEIVES
NATIONAL COMMUNITY HEALTH QUALITY RECOGNITION

(Davenport, IA) – Community Health Care, Inc. (CHC) received multiple Community Health Quality Recognition (CHQR) badges from the Health Resources and Services Administration (HRSA). These awards recognize Health Center Program awardees that have made notable quality improvement achievements in the areas of quality, access, health information technology, and reducing health disparities for the most recent UDS reporting period. CHC has the distinction of being the only health center is the state of Iowa to receive four 2024 HRSA CHQR badges. 

 

Community Health Care, Inc. was awarded the Gold Health Center Quality Leader, Access Enhancer, Advancing Health Information Technology (HIT) for Quality, Health Disparities Reduce badges. CHC was also once again recognized as a Patient Centered Medical Home accreditation.

 

Details for each award:

  • Health Center Quality Leader: Awarded to those achieving the best overall clinical quality measure (CQM) performance among all health centers. Health centers in the top 30% are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), and Bronze (top 21-30%).

 

  • Access Enhancer: Awarded to health centers that earn at least one HCQL or NQL badge, improve by at least 15% in one or more clinical quality measures in back-to-back reporting years. Centers must also increase total patient numbers and the number of patients receiving mental health, substance use disorder, vision, dental, or enabling services by at least 5% in back-to-back years.

 

  • Advancing Health Information Technology (HIT) for Quality: Recognizes health centers that meet all criteria to optimize HIT services. Health centers must meet the following five criteria: adopted an electronic health record (EHR) system, offers telehealth services, exchanges clinical information electronically with key providers health care settings, engages patients through health IT, and collects data on patient social risk factors.

 

  • Health Disparities Reducer: Qualifying health centers must improve by at least 10% in low birth weight, hypertension control, or uncontrolled diabetes CQMs and must improve during back-to-back reporting years for at least one racial or ethnic group. Centers must also perform well or better than the previous year for the quality measures at the health center level. 

 

  • Patient Centered Medical Home (PCMH): Recognizes health centers with PCMH recognition in one or more delivery sites.
