Our CHC Community Health Workers are frontline members of our Population Health Department who are committed to improving the health and wellness of the communities we serve. They establish and maintain relationships with residents and community partners in our region. Our CHWs serve as liaisons between multiple health, social and economic services and the community. They facilitate access to these services and improve the quality and cultural competence of service delivery.
