About CHC

Our Mission

The mission of Community Health Care, Inc. is to provide the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.


 Community Health Care, Inc. (CHC) offers primary, preventive, acute and chronic care, as well as dental, behavior health, OB/GYN,               laboratory, pharmacy, chiropractic, nutrition, and health education services. Without CHC, many patients would  be forced to rely on emergency rooms for their primary care, or they would go without care altogether. In recent years, CHC has made great strides, many of which were made possible with the assistance of generous donors in the Quad Cities area. The number of patients that CHC serves on an annual basis has grown to more than 40,000 individuals, or approximately 10 percent of the Quad City's total population. 


  • Providing medical and dental care to Quad City patients for more than 40 years

  • We serve all patients in need including those with or without insurance, Medicaid and Medicare

  • Our CHC Sliding Fee Program offers lower cost for care

  • We welcome referrals from local hospitals and emergency room departments

  • We have Spanish and Vietnamese translators on staff.  We also offer live video translation for 20 other languages

  • We accept undocumented individuals



Accreditation and Affiliations

We are accredited by the Joint Commission, are recognized as a Patient-Centered Medical Home, and we are designated as a Federally Qualified Health Care Center. All of these showcase our commitment to our mission and to providing our patients with the highest quality of care.

Community Health Care, Inc. receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Patients with concerns about patient care and/or safety within Community Health Care, Inc. are encouraged to contact the CHC Quality Team. If the concerns cannot be resolved by CHC, the patient can contact the Joint Commission's Office of Quality Management to report concerns or register complaints about us by either calling 1-800-994-6610 or emailing complaint@jcaho.org.
